Nothing quite yet. You will receive a second email/letter indicating the number of charts to be reviewed and the month when the chart reviews will be completed. You will receive at least 30 days notice prior to the chart review start date.  

There will be two methods to provide your charts for review: 

1. If your Electronic Health Record (EHR) allows for access by chart reviewers or auditors, then C-WHOs IT team can work with you to provide the appropriate level of access for the specified month of your chart reviews. 
2. If your EHR does not allow access to chart reviewers (or auditors) you will receive a list of documents that will need to be printed to PDF. YCCO’s and/or C-WHO’s IT team will provide access to a HIPAA compliant Sharepoint drive where you will upload those documents. At the end of the review period, these uploaded documents will be digitally shredded.

The number of charts was determined by pre-determined ranges that would provide a statistically reliable representation of the array of services and types of patients that your organization serves, based on the total number clients your organization held sessions with in the year previous.  

Cornerstone Whole Healthcare Organization (C-WHO) is a third party non-profit organization contracted by YCCO to provide unbiased chart reviews. C-WHO and YCCO completed a Business Associate Agreement (BAA), to ensure client confidentiality and agency confidentiality. C-WHO will not disclose agency operations or number of individuals served with other agencies. They will not contact your patients/clients and will abide by HIPAA and HITECH regulations to ensure individual’s privacy.
